Your FirstVisit
Once you have an appointment we will send you a patient registration and health history form. These forms are very important and can be completed in the comfort of your own home. We ask that you mail these back to us if time allows. Otherwise, you may bring them with you to your first visit.
At your first visit we will gather all the necessary information to make a treatment plan to address your periodontal condition. This information will be shared with your general dentist, as we work together to provide you with the best possible care.
Appointments
So that we may assure you and other patient's of having uninterrupted treatment, it is necessary for all patients to accept and adhere to a definite arrangement of appointments and fees. Once an appointment is made, please remember that time is reserved for you.
Insurance & Financial Information
Our financial/insurance coordinator is familiar with most dental insurance companies and plans. We will do your insurance billing for you were possible.
If possible, please provide your insurance information prior to your appointment or bring it with you to your first visit. In most cases you will have a dental co-pay for your exam and treatment, however, we will do everything we can to help eliminate surprises.
It is important to remember that dental insurance is a benefit to help you with your dental costs, but rarely covers all your dental needs, especially as a periodontal patient.
Our primary mission is to deliver the best periodontal care. An important part of that mission is making the cost of optimal care as easy and manageable for our patients as possible. For this reason we offer several payment options. Once a treatment plan is developed our financial coordinator will work with you to determine what will work best for you.
