In most cases patients are referred by their general dentist for a periodontal evaluation. Your general dentist will provide you with a referral slip to our office. In addition, they will provide information to us about your needs with x-rays if they are available. Often additional x-rays will need to be taken to complete your periodontal diagnosis and treatment plan.

If you are new to the area and have seen a periodontist or feel you need to see a periodontist you are more than welcome to call and make an appointment. A referral from a general dentist is not required.

If you are under 18 years of age, you will need to be accompanied by a parent or guardian.
